The document is an application form for assigning a specific low number to a vehicle owned by a Vermont resident. Eligible applicants include family members of the current low number holder, specifically spouse, parent, sibling, child, grandparent, and grandchild. The application requires the applicant's last name, first name, middle initial, license number, mailing address, city, state, ZIP code, phone number, email, vehicle information including current plate number, year, make, model, specific low number request, serial number (VIN), possession of actual low number plates, relationship to assignee, plate type, and previous owner's details. For Conservation or Building Bright Futures plates, a plate fee is due. An individual can have the same number on multiple vehicles registered under the same name(s). The total fees due are stated at the end of the application. Statements and warrants made in the application are certified under Vermont state laws.
