This document is a Bill of Sale and Odometer Disclosure Statement for transferring ownership of a vehicle or vessel. It is issued by the Department of Motor Vehicles in Vermont. The form includes sections for vehicle information, odometer disclosure, seller/transferor information, and buyer information. According to federal and state laws, an accurate odometer reading must be disclosed upon transfer of ownership. Failure to do so may result in fines and/or imprisonment. The document also includes certifications under penalty of law. Signatories are required from both the seller and buyer. For vehicles model year 2011 and newer, self-propelled and with a registered weight of 16,000 lbs. or less, an odometer disclosure statement must be submitted. The document also outlines penalties for selling a salvaged, salvage and rebuilt, or totaled vehicle without disclosing it to the buyer. Additionally, the document indicates that as of January 1, 2021, vehicles of model year 2011 and newer are subject to odometer disclosure for 20 years. Previously, a 2011 vehicle would have become exempt from disclosure in 2021.
