VSA 99 form is used by insurance companies to report the recovery of a stolen vehicle, record the percentage of damage, and estimate the cost of repair. The form requires the insurance company information such as name, telephone number, address, claim payment date, insurance claim number, and company code. Additionally, details about the recovered stolen vehicle are required including its identification number (VIN), year, make, model, actual pre-theft cash value, estimated cost of repair, and actual percentage of damage. The form also asks if there was any water damage and whether a claim of $3,500 or more has been paid by the insurance company for this vehicle. The description of damage is to be indicated in the form if applicable. The certification requires the signature of an insurance company representative, and depending on the condition of the vehicle, different title types may be requested Late model vehicles or those with a cash value of at least $10,000 prior to being damaged require additional checks and fees.
