This form is used when inheriting a vehicle or boat, or receiving it through court decision. Sections to fill out include registration number, Vehicle Identification Number (VIN) or Vessel Hull Identification Number (HIN), year, make, model, type of body, and ownership information. If the deceased did not appoint an executor or administrator, this form must be sent with the title and death certificate. A statement of mileage or release of interest may also be required. The registrant confirms that the registered owner of the vehicle or boat died on a certain date, leaving no estate requiring management or executorship, and that it was not bequeathed to anyone else. This form does not need notarial certification if signed in front of a transport registration office specialist.] [For litigation]: [This certificate takes the place of all other court papers when transferring title for orders from the court (including divorce and probate). Sections include the transferee's name and address, an order transferring title from the case, the administrator's name (if in probate), docket number, and date.
