MV2459 Bill of Sale - Junked Vehicle

This form is used for transferring a vehicle between a registered owner and a licensed salvage dealer, or between two licensed salvage dealers, or from an insurance company when the title has already been surrendered to the Department of Transportation. Required information includes seller and purchaser names, addresses, city, state, ZIP codes, area code, telephone number, sale price, vehicle year, make, identification number or engine number for certain vehicles, and verification of owner's identity through driver license number or photo identification number. The seller affirms that no liens are outstanding against the vehicle and they have the legal right to sell it. If the certificate of title has been lost or destroyed, or marked "JUNKED" and surrendered to the Department of Transportation, then no certificate will be issued for the vehicle.
