This document is a form from the Minnesota Department of Public Safety's Driver and Vehicle Services (DVS) for updating emergency contact information on driving records. The form allows individuals to add, change, or remove emergency contacts. To do so, they must provide their DL number, name, emergency contact name and phone number, and sign the request. Emergency contact information is optional but can be added pursuant to Minnesota Statutes, section 171.12, subdivision 5b. Failure to provide this information means DVS cannot add contacts to a driver's record, preventing law enforcement from notifying anyone in case of an emergency.
